The more information you can provide, the more likely it will be that a full and thorough investigation into your concerns can be conducted.

If you chose to remain anonymous when reporting, please be careful when providing further information that you don't accidentally identify yourself, for instance, by stating your relationship to the person(s) named in your concern.

What should I do if I’ve lost my report key or forgotten the password I created?

Because of the high level of security that is maintained across the Speak Up service, if you lose your report key or password, you will need to raise a new report. Mention in the new report that your concerns relate to a report you raised previously and provide as much information as possible relating to the previous report.

When you create a new report, you will no longer have access to your original report. The case manager will be able to access both records, providing you submit enough detail.
