Report a Concern by Phone
After you complete your report, you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. Use your report key and password to check your report for feedback or questions periodically.
How it works
Concern reported
Submission reviewed, assessed and most appropriate response determined.
Where possible, investigation conducted and/or additional information requested. Reporter contacted regularly with updates.
Outcome determined and appropriate action taken
Reporter informed case is closed
The need for privacy and confidentiality may impact the level of detail provided relating to the investigation or any disciplinary action taken as a result. However, we will keep you informed of the progress of any investigation to the extent appropriate.
