What should I do if I lose my report key or forget the password I created?

Confidentiality is important to us, so if you lose your report key or password, you will need to file a new report. Please reference in the new submission that your concern relates to a previously filed concern and provide as much information as possible relating to the previous report.

When you create a new report, you will no longer have access to your original report. The case manager will be able to access both records, providing you submit enough detail.
