The process for reporting a concern is simple:
- You will be asked a series of questions regarding your concern.
- Next, tell us how we can contact you regarding this concern. If you want to remain anonymous you don’t need to tell us your name. You can still communicate with us through this website by using your Report Key.
- When you submit your report, you will be given a Report Key and asked to create a password. This will allow you to use this website to follow-up on your report. We suggest to check your report for feedback and/or questions after 5-7 business days.
