We greatly value this information and welcome this active engagement that helps us understand community concerns and address any issues swiftly and effectively.

In situations where stakeholders or others need to submit a report in confidence, we are availing this public online platform via this link accessible to everyone internally and externally where reports are received anonymously (if desired by the reporters) and then investigated and managed by HSA for resolution or closure to the best benefit of all in line with our code of conduct and the required compliance to all governing laws and regulations.

To Make a Report

You may use either of the following two methods to submit a report:

After you complete your report, you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
